Evergreen Place Supportive Living offers an affordable assisted lifestyle for both those eligible for Medicaid and those paying privately. We are fully certified by the Illinois Department of Healthcare and Family Services to serve adults of all incomes 65 years of age and older who are seeking the privacy and security of their own apartment with the availability of personal assistance and support services.

The Supportive Living program offered by Evergreen Place provides a highly satisfactory, sensible and cost-effective solution to the growing need for affordable, residential living and support service options for senior citizens. We combine apartment-style living with personal care and other services that are covered by the stateâ€™s Medicaid program. Thanks to this unique state program, Supportive Living residents can live affordably and independently in an environment where personal choice, dignity, privacy and individuality are emphasized nursing care. Services & Amenities

Each resident of Evergreen Place Supportive Living Community is a unique individual with unique care needs. Some seniors need a small amount of assistance while others will need more. Thorough communications between residents, families and loved ones, and our staff ensure that resident receives the appropriate, personal care for his or her needs. To help us better understand individual senior service needs, we conduct an assessment before the resident joins our community. This assessment answers such questions as:

What are our new resident's preferences and needs?
What are his or her individual skills and talents?
What is his or her personal history?

Residents, family, and staff then work together to design a plan that provides the appropriate level of care desired. We help each resident of our senior community reach his or her full potential by providing the right amount of assistance to help make the most of individual capabilities. Our professionally trained staff is available 24 hours per day to provide assistance in the following areas:

Personal Care
Our professional staff stands ready to help residents look and feel their best. Whether a resident needs assistance with bathing, dressing, or grooming, our caring team will gladly assist. These services are always customized to the individual needs of our residents in order to best accommodate their personal preferences and lifestyle choices.

Mobility Assistance
Many of our residents use wheelchairs, walkers, or canes to safely maneuver through our community. Our caring staff is always on hand to provide assistance to residents whether they are going to meals, activities, or just for a walk.

Medication Reminders
Residents may choose whether they want to maintain their own medication administration or to receive reminders and supervision from our staff. We work in conjunction with Green Tree Pharmacy, which will organize medications in planners and provide 24-hour delivery service, free of charge.

Transportation
Evergreen Place offers residents a wheelchair accessible van to provide transportation to and from personal appointments, cultural events, shopping, and community activities. We encourage our residents to stay as active and involved in the community as they desire

Meal Flexibility
Residents enjoy three home-cooked meals each day in our comfortable dining room. Breakfast room service is also available. Meals are served in a restaurant-like format, allowing residents to make selections and enjoy the food they like.

Laundry and Housekeeping
To keep our residents' apartments looking their best, housekeeping and linen services are provided weekly at no extra charge. Residents can use our laundry rooms for their personal belongings, or use our personal laundry service at no additional charge.

Social, Educational, and Cultural Activities
Because our residents have a wide variety of interests and pastimes, we offer several on-site activities each day. We also encourage residents to attend local concerts and health and education programs, as well as dine at their favorite restaurants. Whether you enjoy Bingo or exercising, crafts or cooking, live entertainment or social hours, you will find it all at Evergreen Place.
